Caitlin Bernhard, MSN, FNP is now providing medication management services to support you on your path to radical and empowered wellness. We take a holistic approach to medication management, working with you to create a personalized plan that meets your individual needs. We also collaborate with other healthcare providers, therapists, and family members to provide you with the best possible care. During our medication management sessions, we will: - Review your mental health history, including diagnoses and past and currently prescribed medications - Discuss how these medications are working for you, including side effects, timing of doses, and possible interactions with other medications or supplements - Monitor your progress and process during our follow-up calls/zoom meetings - Connect and collaborate with other specialists and providers - Refer you to other providers as needed/desired Medications we can initiate and help you manage include low-dose naltrexone (LDN), ketamine, anti-depressants, and more. If you are interested in tapering off your prescribed medications, we can discuss the pros and cons of deprescribing as well as what to expect in terms of withdrawal effects and duration of the process. Together we will create a personalized medication plan that ensures that your medications are working well for you. We will provide periodic follow-up care to assess medication effectiveness and adjust for potential side effects. Our goal is to work closely with you to both reduce symptoms of your conditions and reduce potential side effects, allowing you to enjoy your life and feel your best. Our initial session is 45 minutes long and includes a 20-30 minute follow-up call. This will give us time to get to know you and your needs, and to develop a personalized medication plan. The cost of the initial session plus first follow-up is $250. Subsequent follow-up calls are 30 minutes long and cost $100 each. We recommend scheduling follow-up calls every 3-6 months, or more often if needed.
